O: L wing feels stable, can palpate bony callus over the fracture sites. Good extension in wing, wound on ventral aspect of wing almost completely healed. Took x-rays: enough bony callus to feel good about removing pins. Gave 1 mg/kg torb IM. Removed IM pin and cross pins from L ulna. Fractures still feel stable after pin removal, took second x-ray: bony callus bridging fracture sites, still has small areas where bone needs to fill in a little more. Proximal cross pin site bled slightly after removal, held pressure until bleeding stopped.

P: Move to E3D catch, recheck rads next week to see if bird should go outside.

O: Wing fractures and pins feel stable on palpation, able to get good extension in wing. Raised scab on the ventral aspect of the wing - cleaned off to find some SQ tissue protruding out from small wound in skin. Trimmed tissue back, closed site with tissue glue. Took x-rays: not seeing as much bony callus as I would like at the ulna fracture sites. No signs of infection, bones stable with pins in good alignment.

A: Not seeing as much progress at the fracture site on x-rays. May just need to pull pins next week to better assess the area and rule out something like non-union.

Performed PT on wing, slightly reduced extension just due to presence of radial pin at carpus. Took x-rays: hard to see well with pins/epoxy in the way but can see bony callus starting around fracture sites. Pulled radial pin, able to get full extension in wing after pulling pin, radius still feels stable without pin.

A: Patient chewed on L fabric wrap overnight, but did not pull or damage pins. L ulna is stabilizing, but still has some shifting but no grinding felt on palpation. No drainage seen on inside of fabric wrap. AD reviewed last blood results and determined to recheck blood again in a few days at the 2 week mark for current antibiotics. If WBC count is elevated again at next check, then will switch antibiotics.

Took VD/lateral x-rays: pins in place, fragments in alignment, no healing seen yet. Can see chip fragments off the caudal aspect of the ulna fracture site, the proximal fragment of the ulna fracture is in 2 pieces (may be cause of shifting fel), but still in alignment. Cleaned pins sites and applied TAO, replaced fabric wrap.

S: Standing on towel clacking and hissing. Jumped away to back when placed back in cage.

O: Changed L figure 8 wrap and bandages. Noted bandages and gauze padding where soaked through. Applied new telfa/tegaderm bandages over pins and suture sited. Applied L figure 8 wrap through pins. Gave oral/IM meds. Gave fluids in L inguinal. Dropped 60g of pigeon/squirrel/chick.

S: Caught up bird for surgery.

O: See surgery report for details; repaired fractures with IM-ESF tie-in in the ulna and an IM pin in the radius, secured radial pin to cross bar from ulna ESF. Covered dorsal and ventral aspects of wings with telfa/tegaderm and placed figure-8 wrap on wing. No significant bleeding during surgery, but once bird was awake and bandaged, started to see some strike-through on bandages under wrap.

A: Repair went well and bones feels stable after pin placement, but concerned for continued bleeding occurring now that patient is awake and BP is increased.

P: Change wing wrap and bandages in PM, check for continued bleeding for suture lines. Try to place figure-8 wrap tight enough to potentially help stop bleeding but no too tight to cause damage. Give 40 cc LRS SQ in PM as well.

Surgery report
Procedure - Fracture repair - ulna and radius
Performed by - AD
Surgery time - 90 minutes

Pre-medicated with torb IM, anesthetized via isoflurane facemask,
intubated with 3.5 uncuffed ET tube. Removed wrap and bandages
and plucked and scrubbed dorsal and ventral aspects of wing.
 Placed bird in sternal recumbency, incised over the distal radial
fracture site with a #15 blade, dissected through the muscle
to the bone, placed an IM pin in the distal radius and out the
flexed carpus.  Blunted the proximal end of the IM pin, drove
across the middle fragment.  Incised the skin over the second
radial fracture site, found the fracture and drove pin through
the proximal fragment to seat at the elbow.  Drove IM pin up
the proximal ulna, incised over the middle and distal fragment
sites to aid visualization to guide the pin through the middle
and distal fragments to seat at the carpus.  Took intraop x-rays
to confirm placement.  Flushed wounds, closed skin and muscle
with 4-0 webcryl simple continuous sutures.  Turned bird onto
dorsal recumbency, flushed wounds sites on ventral aspect of
wing and closed the muscle and skin layers of three small wounds.
 Returned the bird to sternal recumbency, placed 3 cross pins,
one in each fragment of the ulna.  Bent the IM pins around the
back of the wing, bent the cross pins and secured all pins together
with epoxy.  Covered sutures with telfa/tegaderm on both aspects
of the wing, placed figure-8 wrap on wing.  Minimal bleeding
seen during surgery, but once patient was awake, could start
to see signs of blood on the bandages under the figure-8 wrap.

Left wing wrap in place on L wing but saw blood on bandage material on ventral wing under wrap.

A: Bird bled an abnormal amount from catheter site, continuing to bleed from wound site on L wing. Concerned that bird will bleed during surgery due to rodenticide poisoning causing clotting issues, but fracture needs to be repaired ASAP if bird has any chance of releasability. Prognosis very guarded.

S: Boating, clacking and footy in hand. No feces in cage.

O: Changed wing wrap on L wing. Telfa bandage on ventral aspect of wing covered with blood, but no active bleeding seen on wing. No visible bone seen, covered wound sites with duoderm gel and telfa/ tegaderm. Small wound found on the dorsal aspect of the wing, covered in the same manner, then placed figure-8 wrap on wing. Bird did not absorb SQ fluids from last night, seen as edema on L leg and over abdomen. Scabbing on L foot, R outermost primaries twisted frayed. Bird acting very visual. Drew blood from the R medial metatarsal vein, held pressure for a long time to allow for clotting. Clotted but was very bruised still post draw. Placed IO catheter in the R dorsal ulna - when giving fluids, some leaked into the SQ area on the wing.

A: Mild anemia/hypoproteinemia secondary to blood loss. Bird did not absorb SQ fluids well. Placed a catheter to give IO fluids and to have for surgery - catheter in place but not flowing well, so elected not to try to put bird on fluid pump.

P: Surgery tomorrow. Check catheter and give fluid bolus tonight in PM.

As wound had clotted and no bone exposed, opted not to dig at wound and just applied duogel and covered with telfa and tegaderm. Placed figure 8 wrap on left wing before doing rest of exam.

Feather condition: RP8 twisted and frayed, missing RP5-7 (looks older and not necessarily related to cause of admit)

Radiographic findings: L radius and ulna fractured in multiple places but both should be repairable. Also concerned for extra space at wrist - ulnare bone may have broken off
Initial treatment: 0.18cc Torb IM, 0.16cc Simbadol IM, 0.3cc Meloxicam PO, 1 tab 125mg Clavamox PO, 0.18cc Baytril PO, 0.27cc Pentoxifylline PO, 25cc LRS SQ in right inguinal area

A: Barred owl likely hit by car as found on side of road with fresh left radius and ulna fractures. Fractures should be repairable with surgery and even though likely open at one point are fresh and recovered so infection management should not be a significant issue. Bird in good body condition but likely in shock and suffering from minor head trauma from collision, needs surgery sooner rather than later but would benefit from a day or so of stabilization. Concerned for spacing at left wrist but could be positional or due to swelling, ulnare bone could also be fractured. On right wing bird has a twisted primary feather and is missing 3 primaries but trauma there likely older than cause of admit and bird is in good BCS. Minor ocular trauma not a concern at this time.

P: Rx Meloxicam, pentoxifylline, baytril, Clavamox, calcium slurry, SQ fluids. Tramadol after surgery. Plan for surgery tomorrow if stable, will collect blood, fecal, and IOPs for study. Vet check tomorrow
